Presented in a standing three-quarter view of an elderly white male with the anterior abdominal wall rendered transparent, the alimentary tract is traced from the cervical esophagus through the gastroesophageal junction into the stomach, then distally into the small intestine and onward to the large intestine, rectum, and anal canal. The liver occupies the right upper quadrant, lying superior and lateral to the stomach, and its inferior margin drapes toward the pyloric region. Colonic segments frame the coiled jejunum and ileum, with the transverse colon crossing the upper abdomen and the descending and sigmoid colon coursing inferiorly toward the pelvis. That spatial relationship between hepatic and gastric anatomy is the teaching point most clinicians look for: the left lobe of the liver can overlie the proximal stomach, and the pylorus sits just inferior to the hepatic margin, a frequent source of confusion when correlating surface anatomy with endoscopy or cross-sectional imaging. Age-associated morphology matters here. In older adults, decreased hepatic volume, increased visceral adiposity, and a more ptotic stomach or elongated sigmoid colon can subtly shift organ position, affecting the interpretation of epigastric or right upper quadrant pain and the expected location of tympany, masses, or tenderness on exam.
