Description

Anterior thoracic and upper abdominal wall anatomy is rendered with the intercostal nerves (T1 to T11) highlighted as paired segmental branches coursing laterally from the paravertebral region into each intercostal space along the inferior border of the corresponding rib, where they travel with the intercostal vessels in the costal groove. Superficial musculature frames their pathway, with the intercostal muscles spanning between adjacent ribs and the rectus abdominis positioned medially, its tendinous intersections crossing the linea alba. Laterally, the external oblique forms the flank contour, and the lower thoracic nerves continue anteriorly as cutaneous branches toward the midline and, inferiorly, as thoracoabdominal nerves toward the upper abdomen. Clean color separation keeps nerves distinct from muscle. Understanding this anterior course matters when you need to localize pain or plan a chest wall intervention, because the intercostal nerve runs just deep to the internal intercostal muscle and becomes vulnerable at the inferior rib margin, where intercostal nerve blocks, chest tube insertion, or thoracentesis can injure it if the approach hugs the wrong edge of a rib. Segmental distribution also explains dermatomal patterns, from T1 contributions near the axilla and medial arm region to T7 to T11 supplying the abdominal wall, a common teaching point in herpes zoster and radicular pain.
