Description
Seen in lateral profile, the cervical vertebral column rises from the upper thorax to the occiput, with the spinous processes forming a posterior midline contour deep to the skin of the male neck and upper back. Between adjacent cervical spinous processes, the interspinales (interspinal muscles) occupy the most medial and deepest layer of the intrinsic back musculature, immediately adjacent to the interspinous ligaments and the posterior elements of the vertebrae. Lateral to this midline strip, the epaxial muscle mass thickens into the cervical erector spinae (spinalis, longissimus, and iliocostalis) with deeper fibers of semispinalis and multifidus implied beneath, while the rib cage and shoulder girdle frame the inferior field with scapula and clavicle. Interspinales are small, but they matter for segmental extension control and proprioceptive feedback in the cervical spine, where fine-tuned motion occurs across short lever arms. Pain and spasm after whiplash often localize to the posterior cervical midline, and this view helps separate superficial paraspinals from the deep interspinal and interspinous tissues that can be irritated by repetitive extension, interspinous bursitis, or postoperative scarring after posterior decompression and fusion.
