Superior Mesenteric Vein

Description

Prominent in the mid-abdomen, the superior mesenteric vein ascends within the root of the mesentery, coursing superiorly and slightly to the right of the superior mesenteric artery before approaching the portal venous confluence. Tributaries from the jejunum and ileum converge into mesenteric venous arcades that lie anterior to the lumbar vertebral bodies, with the vein itself positioned anterior to the aorta and medial to portions of the right colon mesentery. Superiorly, the vessel approaches the region of the pancreatic head and neck, where it typically joins the splenic vein posterior to the pancreatic neck to form the hepatic portal vein. Surrounding bony landmarks include the costal margins, lumbar spine, and pelvic brim. An anterior presentation of the superior mesenteric vein matters because it clarifies how venous drainage from the small bowel reaches the portal system and how close this pathway runs to the pancreas and proximal mesenteric root. That anatomic relationship is central in acute superior mesenteric vein thrombosis, where impaired venous outflow can produce bowel wall edema and mesenteric congestion, and it also frames intraoperative decisions during right hemicolectomy or pancreaticoduodenectomy when the SMV and portal vein are exposed, mobilized, or reconstructed.
