Description
Arising from the superolateral calcaneus, the extensor digitorum brevis (highlighted) sits on the dorsolateral aspect of the foot, deep to the skin and superficial to the tarsal bones, with its muscle belly tapering distally into slips that course toward the dorsal aspects of digits two through four. Immediately medial to it, the extensor hallucis brevis is typically visible as a short belly and tendon directed toward the proximal phalanx of the hallux. Beneath and adjacent, the cuboid and lateral cuneiform articulate with the bases of the metatarsals, while the extensor digitorum longus tendons pass more anteriorly and distally to continue into the toe extensor expansions. Orientation and attachment points are emphasized against the metatarsals and phalanges. Clinically, this dorsolateral compartment matters because the deep fibular (peroneal) nerve and dorsalis pedis artery run on the dorsum of the foot, and their relationship to the extensor digitorum brevis is a practical landmark when teaching the dorsalis pedis pulse. Pain or tenderness over the sinus tarsi and dorsolateral foot after inversion injury often raises the differential of extensor digitorum brevis strain or adjacent extensor tendon irritation, and this view clarifies why swelling here can mimic bony tenderness along the tarsometatarsal region.
