Description
Anteriorly on the male left neck, the muscular triangle (trigonum musculare, omotracheal triangle) lies between the midline of the neck medially, the anterior border of the sternocleidomastoid laterally, and the superior belly of the omohyoid superolaterally. Within this space, the infrahyoid strap muscles dominate, with sternohyoid superficial and medial, sternothyroid deeper and more inferior, and the superior belly of omohyoid crossing obliquely toward the hyoid; the thyrohyoid sits superiorly as a short continuation toward the thyroid cartilage. Deep to the strap layer, the laryngeal framework and upper trachea occupy the midline, and the thyroid gland sits anterolateral to the trachea, typically overlapped by sternohyoid and sternothyroid. Clear boundaries matter. Surgical anatomy lives here. The muscular triangle is the working corridor for midline tracheostomy and cricothyrotomy orientation, and it is also the plane where surgeons split or retract the strap muscles to expose the thyroid isthmus and anterior tracheal rings while staying medial to the carotid sheath. Clinically, this region is where thyroid enlargement, thyroglossal duct cysts, and pretracheal lymph nodes present as anterior neck masses that move with swallowing, and where injury to the anterior jugular venous system can complicate superficial dissection.
