Description
Emerging from the anterior rami of the sacral spinal nerves, the nerve to the piriformis is traced from the sacral plexus on the pelvic surface of the sacrum to its point of penetration into the piriformis muscle. Posterior to the sacrum, the paired piriformis bellies span laterally toward the greater trochanter, forming a landmark at the superior margin of the greater sciatic foramen. Inferior to the piriformis, the sciatic nerve descends toward the posterior thigh, while adjacent plexus branches course medially and laterally along the posterior pelvic wall near the ischial spine and ischial tuberosity. Bony reference points include the lower lumbar vertebrae, sacrum, and bilateral ilia, with the proximal femora partially included for hip orientation. Clean relationships. No guesswork. Pinpointing the nerve’s entry into piriformis matters because the muscle is a surgical and anesthetic landmark for the greater sciatic foramen, and its relationship to the sciatic nerve underlies piriformis syndrome and other deep gluteal entrapment patterns. This posterior perspective also supports teaching the anatomic basis of variable sciatic nerve exit (below, above, or through piriformis) and why intramuscular injections in the gluteal region target the superolateral quadrant to reduce sciatic injury. You can also correlate the depicted sacral plexus branching with posterior approaches to the hip and with procedures performed near the ischial spine, where neighboring neurovascular structures are at risk.
