Description
Centered within a semi-transparent thoracic cage, the heart is shown in situ with the apex directed inferolaterally to the left, where a small apical notch interrupts the otherwise smooth contour of the myocardium. The left ventricle forms most of the inferior and left lateral cardiac border and converges on the apex, while the right ventricle lies more anteriorly; the atria sit superiorly at the base, with the right atrium receiving the superior and inferior vena cava. Epicardial coronary arteries course over the ventricular surfaces toward the apex, and the ascending aorta and arch arise superiorly, framed by adjacent mediastinal structures including the trachea. That apical notch functions as a practical landmark when you need to correlate external surface anatomy with internal ventricular topology, because it helps anchor the position of the left ventricular tip relative to the anterior interventricular (left anterior descending) groove and the distal coronary branches that approach the apex. Small shifts in apical position matter. Cardiomegaly from dilated cardiomyopathy can displace the apex inferiorly and laterally, while pericardial effusion can soften the expected apical contour and alter the point of maximal impulse on examination; both scenarios are easier to teach when the normal apical silhouette and its defining notch are explicit.
