Seen from above, the paired thalami sit on either side of the third ventricle, their superior surfaces forming the lateral floor of the body of the lateral ventricles and blending laterally into the internal capsule. The animation organizes the principal thalamic nuclear groups within the diencephalic gray matter, moving sequentially from anterior nuclei near the interventricular foramen toward the medial, lateral, and posterior territories that abut the pulvinar. Boundaries are oriented in standard anatomical planes, with medial nuclei closest to the third ventricle, lateral nuclei positioned adjacent to the posterior limb of the internal capsule, and posterior nuclei tapering toward the midbrain. Subtle transitions clarify how these territories occupy the thalamic core. Thalamic nuclear anatomy is where neuroanatomy becomes clinical localization. Lacunar infarcts in thalamogeniculate perforators can involve the ventral posterior nucleus and produce contralateral hemisensory loss, while lesions affecting the ventral lateral and ventral anterior nuclei disrupt cerebellar and basal ganglia outflow to motor cortex, contributing to ataxia or thalamic dysarthria.
