Description
Thalamic nuclei occupy the dorsal diencephalon as paired ovoid masses bordering the third ventricle, and this superior view emphasizes their internal parcellation within thalamic gray matter. The internal medullary lamina forms a Y-shaped sheet of myelinated fibers that separates anterior and medial nuclear groups from the lateral nuclear complex, with intralaminar nuclei embedded along the lamina. Laterally, the reticular nucleus lies as a thin cap against the internal capsule, while posteriorly the pulvinar expands over the dorsal surface and the geniculate bodies sit at the posterolateral margin in continuity with the optic tract and auditory pathways. Clinical neuroanatomy often hinges on these boundaries. A paramedian thalamic infarct (classically from perforators of the posterior cerebral artery) targets the mediodorsal and intralaminar nuclei and can produce fluctuating arousal, memory disturbance, and vertical gaze abnormalities when the rostral midbrain is involved, whereas a ventral posterolateral lesion interrupts somatosensory relay and may evolve into central post-stroke pain. Discrete nucleus labeling in a fixed superior perspective makes it easier to map vascular territories, correlate sensory or cognitive syndromes with specific relay nuclei, and orient the viewer to the relationship between thalamus and internal capsule. Small structures matter.
