Description
Posterior to the calvaria, the occipital belly of the occipitofrontalis (occipitalis) spans the superior nuchal line and external occipital protuberance, its fibers running superiorly to blend with the galea aponeurotica over the vertex. Laterally, the muscle tapers toward the mastoid region, where its attachment relates closely to the superior nuchal line and the posterior auricular area. The galea forms a broad tendinous sheet superficial to the pericranium, continuous anteriorly with the frontal belly beyond the visible field. Inferiorly, the nuchal region transitions into the upper back, with the trapezius covering the posterior cervical triangle and the deltoid capping the shoulder. Surface anatomy matters here. Elevation of the scalp in craniotomy, occipital flap design, and even posterior hairline procedures rely on the correct plane between galea and loose areolar tissue, because that subgaleal space permits wide spread of hematoma and infection after blunt trauma. Bleeding from scalp lacerations can be brisk when the galea and attached vessels are cut, and the occipital belly’s firm attachments help hold wound edges apart. Nerve considerations are just as real: the greater occipital nerve ascends near the superior nuchal line and may be irritated in occipital neuralgia, while motor supply to occipitalis travels via the posterior auricular branch of the facial nerve. Use this posterior view when teaching scalp layers (SCALP mnemonic), facial nerve branches, and the relationship of the epicranial aponeurosis to adjacent cervical musculature in gross anatomy, head and neck surgery, and anesthetic landmark instruction for occipital nerve blocks.
