Description
Orbital surface of the greater wing of the sphenoid occupies the posterolateral wall of the orbit, viewed from the anterior aspect as if looking into the orbital cavity. Its four-sided plate lies lateral to the orbital apex and posterior to the zygomatic and maxillary contributions, with its superior margin approaching the frontal bone and its posterior edge blending toward the body of the sphenoid. Medially, the surface helps frame the lateral boundary of the superior orbital fissure between the greater and lesser wings, while inferomedially it contributes to the posterior margin of the inferior orbital fissure where the sphenoid meets the maxilla and palatine. Sutural lines and crisp bony edges define these relationships. Lateral orbital wall anatomy matters when you are teaching orbital apex pathways or planning surgical corridors, because the greater wing sits at the junction between orbit, middle cranial fossa, and the superior orbital fissure transmitting cranial nerves III, IV, V1, and VI plus the superior ophthalmic vein. Trauma commonly produces lateral wall or sphenozygomatic fractures, and extension toward the fissure can accompany ophthalmoplegia or sensory loss in the ophthalmic division, a pattern that radiology and maxillofacial teams watch for on CT. Small foramina near the fissural region, when present, can include the meningo-orbital foramen transmitting a recurrent meningeal branch, a detail that influences bleeding risk during lateral orbitotomy.
