The esophagus descends posterior to the tracheal shadow into the stomach, which sits in the left upper quadrant beneath the diaphragm, while the liver occupies the right upper abdomen and partially overlies the gastric antrum. Coils of jejunum and ileum fill the central and inferior abdomen, framed peripherally by the ascending, transverse, descending, and sigmoid colon, with the rectum continuing inferiorly within the posterior pelvis. Superior to the diaphragm, the paired lungs flank the mediastinum and the heart lies anterior to the esophagus and superior to the stomach. A lateral pediatric view clarifies depth relationships that are easy to lose in frontal schematics, such as the retrocardiac course of the esophagus and the way the diaphragmatic domes separate thoracic from abdominal pathology.
