Description
Centered in the anterior neck, the larynx sits superior to the cervical trachea, with the thyroid gland wrapping the upper tracheal rings like a bilobed collar. Each thyroid lobe lies anterolateral to the trachea, joined across the midline by the isthmus, and the laryngeal cartilages occupy the space just inferior to the hyoid region. Posterior depth cues typically include the prevertebral line of the cervical vertebral bodies, while infrahyoid soft tissues frame the airway compartment on either side. Spatial orientation is straightforward. For teaching airway anatomy, an anterior thyroid and trachea relationship matters because it maps directly to common access points and hazards. The isthmus commonly overlies the 2nd to 4th tracheal rings, so a standard tracheostomy window often requires division or retraction of thyroid tissue, and a high-riding isthmus can complicate the procedure. Thyroid enlargement (multinodular goiter) expands laterally and inferiorly and can compress the trachea, producing dyspnea and stridor, while operative dissection near the larynx raises the immediate concern of recurrent laryngeal nerve palsy with postoperative hoarseness, even though the nerve itself is not typically visible from an anterior surface rendering.
