Across the outer surface of the adult male epicranium, the galea aponeurotica (epicranial aponeurosis) stretches as a broad tendinous sheet between the frontal belly of the occipitofrontalis anteriorly and the occipital belly posteriorly. Laterally, this aponeurotic layer blends toward the temporal fascia over the temporal regions, framing the superior aspect of the cranial vault. The frontal bellies lie anterior and slightly inferior to the aponeurosis, while the occipital bellies occupy a posterior, inferior position over the occipital squama near the superior nuchal line. Scalp layers sit superficial to the calvaria. Understanding the epicranium matters because the galea defines the loose areolar plane that permits scalp mobility and also creates a potential space for hemorrhage and infection to spread. In clinical practice, lacerations that violate the aponeurosis tend to gape because the frontal and occipital bellies pull the wound edges apart, a common problem in forehead and vertex trauma requiring layered closure.
