Presented in an anterior orientation, the visceral contents of the true pelvis are arranged from anterior to posterior as the urinary bladder and proximal urethra, the uterus with cervix and upper vagina, and the rectum continuing inferiorly toward the anal canal. Laterally, the broad ligament region frames the uterus, with the uterine tubes extending superolaterally toward the ovaries near the pelvic sidewall, where the internal iliac vessels and ureter typically course. Superiorly, peritoneal reflections define the vesicouterine pouch between bladder and uterus and the rectouterine pouch (pouch of Douglas) between uterus and rectum. Tight anatomy. Clear planes. An anterior pelvic organ view matters when you need to explain how closely the bladder base and distal ureters relate to the cervix and anterior vaginal wall, a relationship that drives risk in hysterectomy, anterior colporrhaphy, and retropubic continence procedures. It also supports teaching of pelvic pain and bleeding patterns, since endometriosis commonly involves the rectouterine pouch and posterior cervix, while pelvic inflammatory disease tracks along the tubes and adnexa and can tether the uterus. These are not abstract spaces, they are operative and symptomatic territories.
