Description
Anteriorly, the fibrous pericardium forms a tough outer cone around the heart, blending superiorly with the adventitia of the ascending aorta, pulmonary trunk, and the intrapericardial portions of the superior vena cava and pulmonary veins. Deep to it, the parietal layer of serous pericardium lines the fibrous sac, while the visceral layer (epicardium) reflects onto the cardiac surface, closely investing the right and left atria and the ventricles. A semi-transparent rendering allows the cardiac chambers to read through the pericardial envelope, with the atria positioned superior and posterior to the ventricles, and the right ventricle lying most anterior. Coronary vessels course in the epicardial fat along the atrioventricular and interventricular grooves. A clean front-facing orientation. This anterior perspective matters when teaching how the pericardium constrains acute cardiac dilation and where it fixes the heart within the middle mediastinum via its diaphragmatic and sternal attachments. Clinically, it supports explanation of pericardial effusion and cardiac tamponade, where fluid in the pericardial cavity restricts right-sided filling first, and it frames the pericardiocentesis approach, commonly performed via a subxiphoid trajectory aimed toward the left shoulder to avoid the internal thoracic vessels and pleura. Clear differentiation of fibrous pericardium, parietal serous pericardium, and visceral pericardium also helps learners understand why pericarditis pain refers via the phrenic nerve (C3-C5) when the parietal layer is involved, while the visceral layer is relatively insensitive.
