Description
Centered on the midline of the upper lip (labium superius), the philtrum appears as a shallow vertical groove bounded by the paired philtral columns that descend from the base of the columella toward the Cupid’s bow. Superiorly, the groove lies immediately inferior to the nasal sill and between the nares; inferiorly it blends into the vermilion border and the central tubercle of the upper lip. An anterior facial view also defines adjacent surface landmarks, including the alar bases laterally and the nasolabial folds as they sweep inferolaterally from the ala toward the oral commissures. A blue marker localizes the philtral depression for rapid orientation. Philtral morphology carries direct diagnostic and reconstructive relevance because it reflects fusion and contouring of the medial nasal and maxillary prominences during embryogenesis. Flattening or effacement of the philtrum is a core dysmorphology feature in fetal alcohol spectrum disorders, and asymmetric or interrupted philtral columns can signal repaired or unrepaired cleft lip.
