Running superiorly from the external carotid artery, the posterior auricular artery is shown coursing posterior to the auricle, then ascending in the groove between the mastoid process of the temporal bone and the cartilaginous ear. From a posterior perspective, the vessel tracks superficially over the insertional region of the sternocleidomastoid on the mastoid process and parallels the posterior border of the auricle as it gives small rami toward the scalp. Occipital bone, upper cervical vertebrae, and the midline spinous processes frame the field, with accompanying veins rendered in blue along the posterior neck. Clear depiction of this artery matters because it is a consistent landmark in posterior auricular and retroauricular approaches, where its caliber and branching can dictate flap design and hemostasis. Ligation or inadvertent injury can complicate mastoidectomy exposure, posterior auricular incisions, or drainage of a postauricular abscess, and it becomes clinically conspicuous in scalp lacerations where retraction of the vessel causes persistent bleeding.
