Posterior leg anatomy is presented from the calf down to the heel, with the superficial posterior compartment dominated by gastrocnemius (medial and lateral heads) overlying soleus and narrowing inferiorly into the calcaneal (Achilles) tendon. The popliteal fossa lies superiorly between the hamstrings and the two heads of gastrocnemius, while the midline calf contour reflects the raphe between the gastrocnemius bellies. Distally, the calcaneal tendon descends centrally and inserts on the posterior surface of the calcaneus, bordered medially and laterally by the softer contours of the triceps surae musculature. Clinical teaching often hinges on this posterior profile because the triceps surae and Achilles tendon are frequent sites of overuse and acute injury. A palpable gap and loss of plantarflexion strength in Achilles tendon rupture, or focal tenderness 2 to 6 cm proximal to the calcaneal insertion in midportion tendinopathy, are best contextualized when the musculotendinous taper and insertional anatomy are clear. This posterior relationship also frames sural nerve and small saphenous vein courses along the posterolateral calf, structures at risk during incisions, endoscopic procedures, or lacerations in the region.
