Description
Viewed from the inferior aspect, the right hepatic lobe is oriented to emphasize the posterior segmental territory of Couinaud segments VI and VII along the posterolateral margin of the liver. Segment VI lies more inferior and lateral, while segment VII extends superiorly and posteriorly toward the diaphragmatic surface, together forming the bulk of the right posterior sector supplied by the right posterior portal pedicle. Along the medial edge of this region, the expected landmark relationships include proximity to the right hepatic vein superiorly and to the groove for the inferior vena cava posteromedially, with the gallbladder fossa and quadrate lobe lying more anterior and medial. Posterior liver, in context. For operative planning, an inferior perspective on segments VI and VII helps when you need to translate surface anatomy into resection planes for a right posterior sectionectomy or a segmentectomy for hepatocellular carcinoma or colorectal liver metastasis. These segments sit deep and posterior, so lesions often relate to short hepatic veins entering the inferior vena cava and to the right hepatic vein, which influences bleeding risk and the order of vascular control during mobilization of the right lobe. Radiology-to-surgery correlation also benefits, since the right posterior sector is defined by portal inflow rather than external lobar boundaries.
