Description
Posteriorly, the upper thorax is presented with the superficial back layer reduced to expose the scapular girdle and the posterior rib cage, while the serratus posterior superior is positioned deep to the rhomboid major and minor along the upper thoracic spine. Its fleshy slips arise from the nuchal ligament and spinous processes around C7 to T3, then pass inferolaterally to the posterior aspects of ribs 2 to 5 near their angles. Medially, the muscle hugs the spinous processes; laterally, it disappears beneath the medial border of the scapula and the overlying scapular retractors, with the ribs forming the inferior and lateral backdrop. This posterior view matters because it clarifies the serratus posterior superior’s frequent confusion with the rhomboids and the erector spinae on exams and in clinical documentation. Palpation and trigger point referral patterns along the medial scapular border often get attributed to rhomboid strain, yet irritation of the thoracic dorsal rami and associated costovertebral joints at ribs 2 to 5 can produce similar posterior thoracic pain and dysesthesia.
