Profile section through the gravid adult female abdomen reveals the uterus occupying the central pelvis and extending superiorly into the lower abdominal cavity at gestational week 15. Within the uterine cavity, the fetus lies in a flexed fetal position, with the head oriented superiorly and the developing upper and lower limbs extending anteroinferiorly relative to the trunk. Anterior to the uterus, the abdominal wall is rendered transparent, while loops of small intestine and portions of the colon are displaced superiorly and laterally by the enlarging uterine fundus. Spatial relationships are clear. Around 15 weeks, the uterus has risen out of the true pelvis, which changes both examination landmarks and operative planning. Clinically, this is the interval when transabdominal ultrasound becomes more informative for fetal biometry (crown rump length transitions to biparietal diameter and femur length), and when the relationship of the uterus to the bowel helps explain patient-reported abdominal fullness and altered bowel habits.
