Description
Depicted from a lateral perspective, the urinary bladder sits superior to the prostate gland (glandula prostatica), with the prostatic urethra passing inferiorly from the bladder neck through the central prostate. Along the posterolateral aspect of the prostatic capsule, laterodorsal lymphatic vessels track posteriorly and superiorly toward the pelvic nodal basins, paralleling the neurovascular tissue that courses on the dorsolateral surface of the gland. Periprostatic venous channels drape over the bladder base and prostate, while autonomic nerve fibers form a fine plexiform network near the bladder neck and posterolateral prostate. Spatial relationships are clear. Laterodorsal prostatic lymphatic drainage matters because it explains early nodal spread patterns in prostate adenocarcinoma, which often reach obturator, internal iliac, and presacral nodes via capsular and periprostatic pathways rather than through the midline alone. During nerve sparing radical prostatectomy, the dissection plane between the prostatic capsule, Denonvilliers fascia posteriorly, and the pelvic plexus posterolaterally forces constant tradeoffs between margin control and preservation of the cavernous nerves responsible for erectile function. That same neighborhood also frames why prostatic inflammation or hemorrhage can track along periprostatic venous and lymphatic routes and complicate pelvic imaging interpretation.
