Description

Running vertically along the anterolateral lumbar spine, the paired psoas major muscles are presented in a female body in anatomical position, with their thick bellies flanking the lumbar vertebral bodies and intervertebral discs. Superiorly, fibers arise from the bodies and transverse processes of T12 to L5 and descend inferolaterally across the pelvic brim, where the psoas fascia forms a distinct investing sleeve. Inferiorly, the muscle narrows to a tendon that passes deep to the inguinal ligament and blends with iliacus to form the iliopsoas tendon inserting on the lesser trochanter of the femur. Surrounding landmarks include the rectus abdominis and external oblique on the anterior abdominal wall, the iliac crest and pelvis medially and inferiorly, and the anterior thigh compartments distally. Few anterior muscular views clarify hip flexion mechanics as cleanly as a well-rendered psoas major in context with the lumbar skeleton and pelvis, because you can track the line of pull from the lumbar vertebrae to the proximal femur without guessing at depth. Clinically, that matters for hip flexor tendinopathy and iliopsoas impingement after total hip arthroplasty, where pain localizes deep in the groin and is provoked by resisted straight-leg raise. The psoas also frames retroperitoneal pathology: a psoas abscess can track along the psoas sheath toward the lesser trochanter, and irritation of the muscle produces the classic psoas sign seen with appendicitis or other retrocecal inflammation.
