Description
Right lateral cardiac contour is defined here without the usual overlay of the great vessels, leaving a clean silhouette of the right atrium as it forms the relatively straight, vertical right border of the heart. Superiorly, this margin would approach the level of the superior vena cava entry point, and inferiorly it tapers toward the region of the inferior vena cava junction and the acute margin near the right ventricle. Anterior to the right atrium, the right ventricle would normally contribute to the sternocostal surface, while posteriorly the atrial wall relates to the interatrial septum and the esophageal plane. No vessels are included. Appreciating the right heart border is not an abstract exercise, it is the reference line used every day when correlating anatomy with a frontal chest radiograph and with cross sectional imaging. Right atrial enlargement from tricuspid regurgitation, pulmonary hypertension, or atrial septal defect classically produces rightward bulging of this contour on CXR, while pericardial effusion tends to expand the overall cardiac silhouette more globally rather than isolating the right border. Stripping away the superior vena cava and pulmonary arterial contours keeps the teaching focus on what actually creates that straight edge. Cardiology and radiology courses use this view when teaching cardiac borders, mediastinal lines, and the anatomic basis of cardiomegaly on chest X ray and CT correlation figures.
