Humeral diaphysis occupies the center of the upper arm between the surgical neck proximally and the supracondylar region distally, with the shaft’s anteromedial and anterolateral surfaces separated by a distinct medial and lateral border. Along the lateral aspect, the deltoid tuberosity interrupts the otherwise smooth cortical contour, while the posterior surface carries the radial (spiral) groove as an oblique channel crossing from medial to lateral. More distally, the lateral and medial supracondylar ridges begin to sharpen as the shaft transitions toward the distal humerus, providing palpable lines that frame the arm’s bony column in a single fixed perspective. A nutrient foramen may be appreciated on the anteromedial surface, oriented toward the elbow. Fracture patterns through the humeral shaft are taught through these landmarks because they predict soft tissue risk and fixation strategy. The radial nerve and profunda brachii artery track in the radial groove, so a midshaft fracture or plate placement along the posterior-lateral cortex raises immediate concern for wrist drop and sensory loss over the dorsum of the hand. Orientation of the deltoid tuberosity and the borders also helps the learner distinguish anterior from posterior humerus when planning anterolateral versus posterior approaches, or when interpreting radiographs and CT reformats that capture only part of the cortex. It matters in the OR.
