Description
Anterior knee osteology is centered on the distal femur articulating with the proximal tibia, with the patella seated in the trochlear (patellar) surface on the anterior distal femur. Medially and laterally, the femoral condyles frame the intercondylar region, while the tibial plateau lies inferiorly with the tibial tuberosity projecting on the anterior proximal tibia. Lateral to the tibia, the fibular head may be seen as a secondary bony landmark, slightly posterior and inferior to the lateral tibial condyle. Bony contours dominate. Clinically, this anterior relationship between the patella, femoral trochlea, and tibial tuberosity anchors assessment of patellofemoral tracking, a common pain generator in active adults and a frequent focus of sports medicine exams. The view also supports teaching the extensor mechanism’s bony line of pull, with the quadriceps acting through the patella to the tibial tuberosity via the patellar ligament, an arrangement relevant to patellar dislocation risk and to tibial tubercle osteotomy planning. Fracture patterns make more sense when you can read the surface anatomy, including transverse patellar fractures and tibial plateau injuries that alter the congruity of the femorotibial joint.
