Description

Anterior abdominal wall transparency exposes the small intestine within a male child, with the duodenum beginning just inferior to the liver and curving posteriorly before transitioning into proximal jejunum. Jejunal loops occupy the left and superior portions of the abdominal cavity, while the ileum courses more inferiorly and to the right toward the ileocecal region. Coiled bowel fills the central abdomen, suspended by mesentery and framed peripherally by the expected position of the colon. Scale and proportions read as pediatric. Orientation matters in pediatrics because the relative abdominal volume, thinner body wall, and shorter mesenteric length make bowel position and distension patterns look different than in adults. A lateral profile is useful when teaching how the duodenum sits retroperitoneally compared with the intraperitoneal jejunum and ileum, and why malrotation can relocate the duodenojejunal flexure and shift small bowel predominantly to the right. Think of the common call for a pediatric upper GI series in suspected volvulus, where knowing the normal course of the duodenum and the typical left-sided position of the ligament of Treitz guides interpretation.
