Description

Along the lateral aspect of the ankle, the distal fibula forms the lateral malleolus, projecting inferiorly and posteriorly to overlap the lateral surface of the talus and the calcaneus at the subtalar joint. Anterior to the malleolus, the anterolateral gutter and sinus tarsi region sit superficial to the talar neck, while posteriorly the retromalleolar groove guides the fibularis (peroneus) longus and brevis tendons as they pass distally toward the foot. The calcaneofibular ligament spans from the tip of the lateral malleolus to the calcaneus, and the anterior talofibular ligament crosses anteromedially from fibula to talus, both lying deep to the superior and inferior fibular retinacula. Skin and subcutaneous contours overlying the lateral malleolus are clearly defined. A common pain generator. Clinically, this lateral view aligns with the typical mechanism of inversion injury, where the anterior talofibular ligament fails first and bony avulsion can occur at the fibular insertion or at the base of the fifth metatarsal during a so-called ankle sprain. Posterior malleolar tenderness raises concern for fibular fracture patterns and for fibularis tendon subluxation, which can follow tearing of the superior fibular retinaculum and present with snapping behind the malleolus. The blue highlight over the lower leg and ankle reads well as a localization marker for injury, injection site planning, or focused examination teaching.
