Description
Positioned at the skull base in lateral profile, the sphenoid bone (os sphenoidale) is highlighted against the surrounding cranial vault and facial skeleton, with its greater wing forming a broad plate just posterior to the orbit and anterior to the temporal region. Anteriorly, the sphenoid articulates with the frontal, zygomatic, and maxilla along the lateral orbital wall, while posteriorly it meets the temporal bone near the squamosal suture and continues medially toward the clivus. Inferior to the greater wing, the pterygoid process descends toward the nasopharyngeal space, projecting inferiorly and slightly anteriorly from the body of the sphenoid. The lesser wing curves superiorly toward the midline and frames the region of the superior orbital fissure in relation to the orbital apex. A lateral view is the fastest way to orient the sphenoid’s external landmarks that guide endoscopic and open approaches to the middle cranial fossa. The greater wing contains the foramina (ovale and spinosum) transmitting the mandibular division of the trigeminal nerve and the middle meningeal artery, and the pterygoid region anchors the medial and lateral pterygoid muscles, a frequent point of reference when teaching masticator space spread of odontogenic infection. Small bone, big consequences. Although the sella turcica is a midline feature and less conspicuous laterally, its relationship to the sphenoid body explains why transsphenoidal routes can reach pituitary pathology while avoiding a formal craniotomy.
