Description
Situated at the medial wall of the second part of the duodenum, the hepatopancreatic ampulla (ampulla of Vater) forms where the common bile duct joins the main pancreatic duct before opening at the major duodenal papilla. Encircling this terminal channel, the sphincter of the ampulla (sphincter of Oddi) appears as a discrete ring of smooth muscle, with the biliary and pancreatic duct segments lying proximal to it and the duodenal lumen distal to it. Superior and lateral, the extrahepatic biliary tract is implied by continuity toward the gallbladder and hepatic ducts within the hepatoduodenal ligament (Glisson’s capsule and the Glissonian sheath providing the named connective tissue context). Orientation is external, emphasizing the muscular collar rather than mucosal relief. Functional anatomy drives the clinical relevance here. Spasm or stenosis of the sphincter of Oddi can obstruct bile and pancreatic juice outflow, producing biliary-type pain, postcholecystectomy symptoms, and episodes of pancreatitis when pancreatic duct pressure rises. Endoscopic cannulation during ERCP targets the major papilla and traverses this sphincter, so its position relative to the distal common bile duct and pancreatic duct matters when interpreting difficult duct access, contrast hold-up, or post-procedure pancreatitis risk. A small structure, a big consequence.
