Description
Arising from the posterior surface of the manubrium sterni and the first costal cartilage, the sternothyroid muscle ascends in the anterior neck deep to sternohyoid and inferior to the thyroid cartilage. Its fibers run superiorly and slightly laterally to insert along the oblique line of the lamina of the thyroid cartilage, placing it medial to the sternocleidomastoid and anterior to the cervical vertebral column. A midline interval between paired infrahyoid muscles is implied, with the laryngeal framework superior to the thoracic inlet and clavicles. Orientation is straightforward. Functionally, sternothyroid is a laryngeal depressor that draws the thyroid cartilage inferiorly after swallowing and during phonation, counterbalancing the suprahyoid and thyrohyoid elevators. This anterior perspective is the one surgeons and trainees rely on when thinking through the strap muscle layers encountered in tracheostomy, thyroidectomy, and anterior approaches to the larynx, where midline separation or division of infrahyoid muscles can improve exposure. Injury or denervation in the ansa cervicalis distribution can alter laryngeal positioning and voice, a subtle point that becomes clear when you can track the muscle directly onto the thyroid cartilage.
