Across the sequence, the camera tracks over its broad, flattened cortical surface and then sweeps medially and laterally to relate the lobule to the adjacent simple lobule, the posterior quadrangular lobule, and the intervening cerebellar fissures. Foliated gyri and sulci come into relief as the hemisphere rotates from a more dorsal to a slightly posterolateral viewpoint. Orientation to the quadrangular lobules matters whenever you are localizing cerebellar lesions by surface anatomy, whether on MRI correlation, operative notes, or an anatomy practical. The anterior quadrangular lobule participates in the spinocerebellar and corticocerebellar circuitry that calibrates limb coordination, so focal injury here often tracks with ipsilateral appendicular ataxia and dysmetria.
