Description
Running inferior to the twelfth rib, the subcostal nerve (anterior ramus of T12) courses laterally from the thoracolumbar region and crosses the quadratus lumborum on its anterior surface before continuing toward the anterolateral abdominal wall. Posteriorly, its origin relates to the T12 spinal nerve as it exits the intervertebral foramen, with the rib cage and thoracolumbar vertebral bodies forming the bony backdrop in lateral profile. Inferior to the lower ribs, the iliac crest and superior pelvis provide a landmark for the nerve’s descent as it approaches the plane between the internal oblique and transversus abdominis. Short segment, long reach. A lateral presentation clarifies why the subcostal nerve is grouped with the thoracoabdominal nerves yet behaves differently from typical intercostals, because it runs beneath the last rib and traverses the posterior abdominal wall rather than occupying a true intercostal space. This is the anatomy behind postoperative flank or lower abdominal wall numbness after retroperitoneal exposures, nephrectomy incisions, or lateral trocar placement, where T12 injury can produce sensory loss over the suprapubic and lateral gluteal region and contribute to abdominal wall weakness. It also frames the relationship to the quadratus lumborum, a common target for QL blocks, where spread can involve T12 and alter dermatomal coverage.
