Superior Rectal Nerve, Posterior View

Description

Seen from a posterior perspective in the male pelvis, the rectum descends in the midline from the sigmoid colon toward the anal canal, with the superior rectal nerve coursing along its posterior surface within the mesorectal fat. Fine autonomic branches converge from the hypogastric plexus region and distribute circumferentially to the rectal wall, tracking inferiorly toward the pelvic floor. Small ganglionic elements and plexiform interconnections are suggested along the nerve’s path, consistent with the continuity between prevertebral (superior hypogastric) input and the pelvic (inferior hypogastric) plexus. Posterior views of rectal innervation matter because they mirror the operative corridor used in total mesorectal excision and posterior mobilization of the rectum. Injury to sympathetic fibers traveling via the hypogastric nerves and pelvic plexus can contribute to postoperative bowel dysfunction and, in men, sexual side effects through nearby autonomic pathways to the prostate and seminal vesicles. Clean planes. Blunt traction or thermal spread near the posterolateral mesorectum is where these fibers are commonly compromised. Use this artwork to teach pelvic autonomic anatomy in colorectal surgery modules, to accompany figures on defecatory physiology (rectoanal inhibitory reflex, rectal compliance, and sensation), or to clarify nerve-sparing principles in oncologic rectal resection and pelvic dissection.
