Description
Anterior to the auricle, the male temporal region is defined by the contour of the temporalis muscle and its overlying fascia, with the zygomatic arch marking the inferior boundary and the superior temporal line curving superiorly onto the parietal bone. The pterion lies a few centimeters superior and slightly anterior to the external acoustic meatus, where frontal, parietal, sphenoid (greater wing), and temporal bones meet. Superficial soft tissue padding follows the temporal fossa, while the temporalis tendon courses deep to the zygomatic arch toward the coronoid process of the mandible. Key surface planes stack from skin to subcutaneous fat, then the temporoparietal fascia (continuous with the SMAS), the loose areolar layer, and the deep temporal fascia investing temporalis. Palpation and injection in this area are unforgiving. The pterion overlies the anterior division of the middle meningeal artery; a blow here can produce an epidural hematoma even when the scalp looks benign. For facial surgery and aesthetic procedures, the temporoparietal fascial plane matters because the frontal branch of the facial nerve travels within or just deep to this layer as it crosses the zygomatic arch, and the superficial temporal artery and vein provide both a landmark and a hazard for temple filler, biopsy, and flap design.
