Description
Petrous part of the temporal bone dominates the frame, oriented in superior view so its anterior surface forms the lateral floor of the middle cranial fossa. Along the petrous ridge, the sharp superior border runs obliquely, separating the middle cranial fossa from the posterior cranial fossa and guiding the expected course of the superior petrosal sinus. Medially, the petrous apex points anteromedially toward the region of the foramen lacerum, while more laterally the arcuate eminence rises on the superior surface above the anterior semicircular canal, with the tegmen tympani lying anterolateral as a thin bony roof over the tympanic cavity and mastoid antrum. Anteromedial to the arcuate eminence, the trigeminal impression (impressio trigemini) marks the site of the trigeminal ganglion in Meckel’s cave. A hard surface. Skull base surgery and temporal bone radiology both hinge on these landmarks because they locate neurovascular corridors that are narrow and unforgiving. The greater petrosal nerve runs in the hiatus and groove on the anterior petrous surface toward the foramen lacerum and pterygoid canal, so this fixed superior perspective supports teaching why anterior petrosectomy and middle cranial fossa approaches must respect the geniculate ganglion region and the thin tegmen to reduce facial nerve injury and postoperative cerebrospinal fluid leak. Arcuate eminence morphology also matters when correlating surgical anatomy with CT, including discussion of superior semicircular canal dehiscence and why the eminence is an indirect, not infallible, surface marker.
