Description
Arising as a firm dural reflection, the tentorium cerebelli forms a sloping shelf between the occipital lobes of the cerebrum superiorly and the cerebellar hemispheres inferiorly, presented from a lateral perspective with a mid-sagittal section through the male brain. Medially, its free margin curves anteromedially toward the tentorial notch, framing the dorsal aspect of the midbrain as it transitions into the brainstem, while its attached border blends with the cranial dura along the posterior cranial fossa. Nearby midline landmarks in the cut plane include the corpus callosum arching above the diencephalon, the thalamus and hypothalamus around the third ventricle, and the pituitary gland seated inferiorly at the sella region. Clear spatial separation. This view matters because the tentorium defines the compartmental anatomy that underpins transtentorial (uncal) herniation, where medial temporal lobe tissue is displaced inferiorly through the tentorial notch and can compress the cerebral peduncle and ipsilateral oculomotor nerve. It also mirrors the operative corridor and constraints faced in posterior fossa and supratentorial approaches, where the relationship between the cerebellum, brainstem, and dural folds dictates safe retraction and venous preservation at the tentorial edge and adjacent sinuses. Teaching this relationship early prevents sloppy mental models.
