Centered on the third pedal digit, the dorsal surface of the toe is presented with the nail plate and eponychium at the distal end and the dorsum of the proximal, middle, and distal phalanges aligned proximodistally toward the metatarsophalangeal (MTP) joint. Lateral to the second toe and medial to the fourth, the third toe’s dorsal skin creases mark the proximal and distal interphalangeal (PIP and DIP) joints, where the extensor mechanism lies immediately deep to the dermis. A blue toe separator occupies the second interdigital space, maintaining abduction between the second and third digits at the level of the proximal phalanges.
