Across the anterior (palmar) aspect of a male hand, the transverse head of the adductor pollicis is emphasized as a horizontal muscle belly spanning the deep palm from the third metacarpal region toward the ulnar base of the proximal phalanx of the thumb. Superficial and radial to it, the thenar musculature lies in the eminence, while long flexor tendons from the forearm continue distally toward the digits within the palmar compartment. Blue vascular elements track along the palm in the expected course toward the thumb and fingers, and pale fibrous bands suggest the palmar aponeurosis and related connective tissue stabilizing the flexor mechanism. Orientation is clear. This anterior perspective matters because the transverse head forms the muscular floor deep to the flexor tendons and sits adjacent to the adductor pollicis insertion complex at the ulnar side of the thumb, a key contributor to pinch strength at the metacarpophalangeal joint. Injury to the deep branch of the ulnar nerve within Guyon’s canal, or more distally in the deep palmar compartment, can denervate the adductor pollicis and produce Froment’s sign, with compensatory flexion at the thumb interphalangeal joint driven by flexor pollicis longus. Hand surgeons also use these relationships when planning approaches around the first web space and when distinguishing tendon excursion problems from intrinsic muscle dysfunction.
