Rendered from a posterior (dorsal) view of a female child, the thoracic cavity is opened visually through the integument to orient the lungs on either side of the vertebral column, with their inferior margins approaching the diaphragm and posterior costal recesses. Inferior to the diaphragm, the abdominal viscera occupy the upper abdomen while the retroperitoneal kidneys sit high and lateral to the lumbar spine, their hila facing anteromedially. Ureters descend from each renal pelvis along the posterior abdominal wall toward the pelvis, terminating in the urinary bladder, which lies more anterior but remains referenced in its midline pelvic position. Spatial relationships read cleanly. No distractions. A posterior pediatric orientation matters because many high-yield clinical landmarks are assessed from the back: breath sounds over the posterior lung fields, pleural effusions layering dependently, and lower lobe pneumonias presenting with posterior basilar findings on auscultation and imaging. The kidneys are best contextualized dorsally as retroperitoneal organs protected by ribs 11 and 12, a relationship that explains flank tenderness in acute pyelonephritis, the pain pattern of nephrolithiasis radiating from costovertebral angle to groin, and the planning of posterior approaches for renal biopsy or percutaneous nephrostomy to minimize transperitoneal injury. It also reinforces how diaphragmatic excursion can shift apparent organ positions during respiration. Clinicians teach from this view.
