Description
Ventral anterior (VA) nucleus occupies the rostral portion of the ventral tier of the thalamus, positioned at the anterior end of the lateral nuclear group in a fixed anterior perspective. Medially, the thalamic mass approaches the midline structures associated with the third ventricle, while laterally the VA nucleus lies adjacent to the internal capsule, the key white matter boundary separating thalamus from the lentiform nucleus. Superior and posterior to VA, the ventral lateral nucleus and other lateral thalamic territories continue caudally, and anteromedially the region transitions toward the anterior nuclear area at the thalamic pole. Borders matter here. Functionally, VA is part of the motor thalamus, receiving major inhibitory input from the globus pallidus internus and substantia nigra pars reticulata and projecting to premotor and supplementary motor cortices, so its location helps explain why small paramedian or thalamogeniculate territory infarcts can produce disproportionate deficits in motor planning and initiation. The anterior viewpoint emphasizes VA’s relationship to the anterior limb and genu region of the internal capsule, a practical orientation when correlating with MRI slices and stereotactic coordinates used in functional neurosurgery. It also helps separate VA from neighboring ventral lateral targets more often referenced in tremor surgery.
