Zona incerta occupies the subthalamus as a narrow band of gray matter, positioned inferior to the thalamus and superior to the subthalamic nucleus within an anterior-facing section at diencephalic level. In this slice, the zona incerta lies between the thalamic fasciculus (field H1 of Forel) dorsally and the subthalamic nucleus ventrally, with its medial margin approaching the hypothalamus and its lateral aspect bordering the internal capsule and adjacent pallidothalamic fiber systems. Nearby landmarks you would expect in the same plane include the ventral thalamic nuclei superiorly and the cerebral peduncle more inferolaterally, helping orient the viewer to the rostral midbrain and diencephalic junction. Small structure, big consequences. For functional neurosurgery, the zona incerta matters because it is a frequent target or “neighbor-at-risk” during deep brain stimulation planning for tremor syndromes, including essential tremor and Parkinsonian tremor, where stimulation near the posterior subthalamic area can modulate cerebellothalamic outflow. The fixed anterior section clarifies why lead placement can inadvertently engage pallidothalamic fibers in the fields of Forel or the subthalamic nucleus, a common source of capsular side effects such as dysarthria or facial pulling when current spreads laterally.
