Posterior views of both lower limbs are presented for side-by-side comparison, with the left leg serving as a normal reference and the right leg demonstrating extensive cutaneous and subcutaneous changes over the posterior calf. Discoloration concentrates along the sural region overlying the gastrocnemius and soleus compartments, extending inferiorly toward the calcaneal (Achilles) tendon and superiorly toward the popliteal fossa, with patchy areas of violaceous to black necrosis surrounded by erythematous inflammation. The distribution implies deep involvement along the crural fascia (fascia cruris) rather than an isolated superficial dermatosis. Skin findings dominate, but the anatomic target in necrotizing fasciitis is the deep fascia. Necrotizing fasciitis is a surgical emergency because bacterial toxins and thrombosis of perforating vessels can drive rapid fascial plane destruction before surface changes appear dramatic, and the posterior compartment of the leg provides a long, contiguous pathway for spread. Pain out of proportion, systemic toxicity, and evolving ecchymosis or bullae often precede frank necrosis, and delay increases the risk of limb loss and septic shock. Surgical debridement typically follows longitudinal incisions that allow blunt exploration along the fascia cruris and between muscle compartments, with attention to the popliteal neurovascular bundle proximally and the posterior tibial and peroneal neurovascular structures distally. Time matters.
