Description
Cut in transverse section and viewed from a superior aspect, the heart is opened to the level where the right and left ventricles lie anteriorly and the atrial cavities sit more posterior. Along the anterior (sternocostal) surface, the anterior interventricular sulcus, also termed the anterior interventricular groove or paraconal groove, marks the external plane of the interventricular septum between the ventricular chambers. From this orientation, the sulcus runs longitudinally toward the cardiac apex, positioned medial to the left ventricular free wall and lateral to the right ventricular outflow region. That surface landmark matters because it guides you directly to the course of the left anterior descending artery (anterior interventricular branch of the left coronary artery) and its companion great cardiac vein, structures implicated in common anterior wall and septal myocardial infarction. It is also a practical reference during coronary artery bypass grafting and in surgical or CT correlation of ventricular septal defects and aneurysmal remodeling after LAD occlusion. For teaching, it clarifies the difference between the internal interventricular septum and the external sulcus that overlies it. A frequent point of confusion.
