Description
Suspended in flowing plasma, an enveloped West Nile virus virion dominates the field, its lipid membrane studded with surface glycoproteins consistent with the flaviviral E protein arrangement. Surrounding it, erythrocytes (red blood cells) appear as biconcave discs in mixed orientations, some edge-on and some face-on, creating depth within the vessel lumen. Smaller formed elements suggest platelets and occasional leukocytes in the background, with the overall scene framed by the dark, tubular interior of a blood vessel wall. Viremia is the transient phase that links an initial mosquito-borne inoculation in skin and lymphatics to systemic spread and, in a minority of patients, neuroinvasive disease. Seeing the virion in the bloodstream helps explain why nucleic acid amplification testing of donated blood reduces transfusion-transmitted West Nile virus and why immunocompromised patients can sustain higher viral loads.
