Radiographic rendering traces the female spinal cord from the foramen magnum into the vertebral canal, aligned posterior to the vertebral bodies and anterior to the laminae and spinous processes. As the sequence advances caudally, the cord tapers into the conus medullaris (typically near L1 to L2 in adults) and transitions into the cauda equina, a bundle of lumbar, sacral, and coccygeal nerve roots descending within the lumbar cistern. Segmental spinal nerves are implied by root exit toward the intervertebral foramina, with cervical levels appearing more compact and lumbar roots elongating progressively inferiorly. Orientation matters clinically. The animated caudal “fly-through” clarifies the mismatch between vertebral level and spinal cord segment, a persistent source of error when correlating neurologic deficits with CT or plain-film landmarks. Seeing the conus and cauda equina in motion also helps explain why lumbar puncture and spinal anesthesia target the L3 to L4 or L4 to L5 interspace, below the end of the cord but within the cerebrospinal fluid space surrounding the nerve roots. Conus medullaris syndrome versus cauda equina syndrome reads differently on exam and on imaging, and this radiograph-style progression makes that distinction easier to teach.
