Description
Beginning with an anterior pelvic view, the uterus is centered between the urinary bladder anteriorly and rectum posteriorly, with the cervix inferior and the fundus superior, and the paired uterine tubes extending laterally toward the ovaries. Over the animation sequence, ectopic endometrial implants appear on the serosal surface and adjacent peritoneum, evolving from punctate powder-burn lesions to fibrotic plaques that tether the uterus to the broad ligament, adnexa, and cul-de-sac (pouch of Douglas). Ovarian involvement progresses into an endometrioma, a thick-walled cyst on the ovary’s medial aspect that expands and distorts the tubo-ovarian relationship. Adhesions tighten. Endometriosis matters because these surface and subserosal changes translate directly into symptoms and operative findings, from cyclic dysmenorrhea and deep dyspareunia to chronic pelvic pain and subfertility due to impaired fimbrial capture and altered tubal motility.
