Description
Spanning the posterior trunk from the inferior ribs to the iliac crests, the thoracolumbar fascia (lumbodorsal fascia) forms a broad aponeurotic sheet over the lumbar region, draping across the midline spinous processes and extending laterally toward the abdominal wall. Inferiorly, it approaches the posterior superior iliac spines and the sacral region, where its dense fibers blend with the sacral fascia and the superior margin of the gluteal fascia overlying gluteus maximus. Superficial contours of the lumbar paraspinal mass are suggested deep to the fascial layer, with the upper gluteal region and the bilateral gluteus maximus bellies occupying the inferolateral field. Clear midline orientation. For teaching biomechanics and pain patterns, a posterior view of the thoracolumbar fascia matters because it is the interface where trunk extensors and abdominal wall tensions couple across the lumbar spine, with attachments that also transmit load to the pelvis via the iliac crest and sacrum. Clinically, this is the tissue plane encountered during posterior lumbar approaches and the region implicated in some cases of nonspecific low back pain, where fascial thickening, altered glide over the erector spinae, or altered load transfer around the sacroiliac region can be discussed with anatomical specificity. It also frames the superior boundary of the posterior iliac crest, a common landmark for bone graft harvest and posterior iliac crest injections. Use this illustration in gross anatomy and kinesiology modules when you need to distinguish superficial fascia, thoracolumbar aponeurosis, and underlying paraspinal muscle compartments, or in surgical and interventional training materials covering posterior lumbar exposure and landmark-based procedures around the PSIS and iliac crest.
