Description
Posterior trunk anatomy is rendered with the thoracolumbar fascia emphasized as a broad aponeurotic sheath spanning the lower thoracic and lumbar regions. Superficial layers include trapezius superiorly and latissimus dorsi sweeping inferolaterally toward the humerus, while the fascial laminae lie deep and medial, tethered along the lumbar spinous processes and the posterior iliac crest. Paraspinal muscle columns (erector spinae and multifidus) occupy the groove immediately lateral to the vertebral column, with scapular margins and thoracolumbar vertebrae providing bony reference points. Blue-toned elements read as deeper connective tissue planes and attachment lines against the red muscle masses. Thoracolumbar fascia matters because it is the mechanical interface between latissimus dorsi, gluteus maximus, and the lumbar extensor compartment, transmitting load across the lumbopelvic region during lifting, gait, and trunk rotation. In posterior approaches to the lumbar spine, the plane between the fascia and the erector spinae guides midline exposure, while its dense attachments at the spinous processes and iliac crest help orient dissection. Low back pain patterns often track this anatomy, including strain at the iliac crest attachment and altered force coupling across the posterior oblique sling (latissimus dorsi to contralateral gluteus maximus via the thoracolumbar fascia).
